Well yes, except that "ti chiami" is the second person singular indicative of chiamarsi, meaning "your name is". The third person would be "lui/lei si chiama". Other than that, I totally agree that he could have used that verb, I don't know why I didn't mention it :-p
Like I said, I only know basic Italian, so I honestly wouldn't know which of the two would be more appropriate for an epitaph :-p
